Patient Agreement

This agreement outlines the responsibilities and expectations of patients receiving care from Vertis Health. These requirements help us provide safe, effective, and timely treatment and ensure compliance with prescribing and clinical governance standards.

By engaging with the service, you agree to the following expectations.

Patient Agreement Terms

  • Attending Scheduled Appointments

    You are responsible for attending all scheduled appointments, including assessment, titration, medication review, and follow-up appointments.

    If you are unable to attend your appointment, please notify the service at the earliest opportunity so that we can reschedule your appointment and offer the slot to another patient.

    During all appointments, you must be located in a private, confidential, and appropriate environment where you are able to communicate without interruption, maintain privacy, and access a reliable phone or internet connection sufficient to participate in the appointment.

    Missed Appointments

    Should you miss your appointment, we cannot guarantee when the next available appointment will be. Due to the clinical monitoring requirements associated with ADHD medication, we may be unable to issue further prescriptions until you have been reviewed during a titration or medication review appointment.

    Repeated missed appointments may result in discharge from the service in accordance with our non-attendance policy.

  • Blood Pressure and Pulse Readings

    You are expected to upload blood pressure and pulse readings prior to each appointment when requested by your clinician.

    These readings are required to support safe prescribing and medication monitoring.

    Failing to provide blood pressure and pulse readings before your appointment could result in delays to prescribing decisions and the issuing of prescriptions.

    Where appropriate, additional physical health information may also be requested.

  • Safe Use of Medication

    You are responsible for:

    • Taking medication exactly as prescribed.

    • Following any titration instructions provided by your clinician.

    • Storing medication securely and safely.

    • Ensuring medication is not shared with any other individual.

    • Informing the service if you stop taking your medication.

    Prescription Requests

    Prescriptions can only be issued when clinically appropriate and when required monitoring information has been provided.

    Prescription requests should be submitted within the timescales advised by the service. Late requests may result in delays.

    Lost, stolen, damaged, or unused medication may not be replaced and will be considered on a case-by-case basis.

  • You must inform the service promptly if:

    • You experience side effects or adverse reactions.

    • Your physical health changes significantly.

    • You are admitted to hospital.

    • You become pregnant or are planning a pregnancy.

    • You start, stop, or change any other medications, including over-the-counter or herbal treatments.

    • You develop any new medical conditions that may affect ADHD treatment.

    If you experience severe side effects or a medical emergency, seek urgent medical attention immediately.

  • To help us provide effective care, you agree to:

    • Respond to requests for information within a reasonable timeframe.

    • Provide accurate and up-to-date contact details.

    • Inform us promptly if your address, telephone number, email address, or GP details change.

    • Check communications from the service regularly.

    Failure to respond to repeated requests for information may affect ongoing treatment and prescribing.

  • Where appropriate, information relating to your ADHD assessment, treatment, medication, and monitoring may be shared with your GP and other relevant healthcare professionals involved in your care.

    Patients are expected to engage with any agreed shared care arrangements and routine health monitoring requirements.

  • We are committed to providing a safe and respectful environment for patients and staff.

    Patients are expected to communicate respectfully with all members of the service.

    Abusive, threatening, discriminatory, or inappropriate behaviour towards staff may result in restrictions to service access or discharge from the service.

  • Patients may be discharged from the service if they:

    • Repeatedly fail to attend appointments.

    • Repeatedly fail to provide required monitoring information.

    • Do not engage with treatment recommendations.

    • Cannot be contacted despite reasonable attempts by the service.

    • Present concerns regarding the safe continuation of treatment.

    Where possible, patients will be informed of any discharge decision and advised of the next steps.
