Learn how our service intersects with the care offered by your GP.

What is Shared Care?

A Shared Care Agreement is an agreement between a service and your GP practice to share information about you, in order to give you better, safer care. Often, different service providers will use different computer systems to record your details, and the Shared Care Record allows providers to share these details to better understand your needs and make the best decisions with you, and for you.

If it is agreed by your GP practice, we will send a letter to your GP practice including the information outlined below, and your GP will overtake the prescribing responsibilities for your medication.

Transition to Shared Care

Once you are ready to exit to Shared Care, we will send a Summary Letter to your GP signposting your practice to your personal record, including full information on consultations and medications. The letter will also include:

  • Your name, date of birth and NHS number

  • Any prescription drug names, brands, formulations, dosage frequency and timings, and the dates you started any medications

  • The length of time the dose has been stable for, and how long you’ve had benefits from the medication

  • The date from which care should be transferred

  • Any monitoring requirements

  • Info on the pathway to refer back to Vertis Health

Once a Shared Care Agreement is accepted by us and your GP, we will issue a final four-week prescription. After this point, your prescriptions will be handled by your GP practice.
