A guide for clinicians

About this service.

The Enhanced Primary Care Hubs for Adult ADHD Reviews are commissioned by NHS Herefordshire & Worcestershire to support Primary Care in delivering safe, compliant annual reviews for adults diagnosed with ADHD.

This service is delivered by Vertis Health—Worcestershire’s GP Federation—and led by a team of UKAAN-trained clinicians working in close partnership with local practices.

Who can be referred to this service?

This service is suitable for patients who:

  • Are aged 18 or over

  • Are registered with a GP in Worcestershire

  • Have a confirmed diagnosis of ADHD (childhood or adult)

  • Are currently prescribed ADHD medication

Please note: This is a review service. New assessments and diagnosis are not part of this pathway.

How can I refer a patient into this service?

As this programme launches, we will be in contact with practices and PCNs across Worcestershire to request lists of patients eligible for this service, using a preset EMIS search template.

Eligible patients may be provided in bulk through this format - with Vertis Health then making contact with those due an annual review over the course of the year.

If you would like to refer a patient on a one-off basis, you can do so via eRS.

For any clinical concerns where a rapid ADHD review is required, GPs can refer via eRS and select Urgent. These will be prioritised within 5 working days.

We also maintain open communication lines via:

  • Email, to the usual Vertis Health inbox

  • Direct telephone booking line for patient or practice queries [to follow]

The Enhanced Primary Care Hubs for Adult ADHD Reviews are built to support the NHS Herefordshire & Worcestershire Shared Care Agreement, ensuring a structured, safe, and collaborative model for ongoing ADHD treatment in primary care.

Vertis Health acts as your clinical support partner—delivering high-quality annual ADHD medication reviews on behalf of practices. Here’s what that means in practice.

Comprehensive Medication Reviews

Our clinicians are:

  • UKAAN-trained in ADHD diagnosis, treatment and prescribing

  • GP-led, ensuring local expertise and relevance

  • Supported by robust EMIS-based templates to standardise care and record outcomes into your patient records in real time

Each review includes:

  • Full side effect discussion (inc. hepatic and psychiatric warnings)

  • Review of contraindications and comorbidities

  • Cardiovascular risk checks and BP/pulse monitoring

  • Review of seizures, tics, sleep, and other psychiatric symptoms

  • Lifestyle and compliance discussion

  • Clear documentation and outcome summary recorded directly into EMIS

Prescribing - what we do, and what we don’t

If during review:

  • Medication is deemed inappropriate or not tolerated

  • New side effects or contraindications emerge

  • Patient preference or clinical need prompts a change

…our prescribers will initiate titration (if appropriate), following HWICB pathways. You will be updated at each stage, and prescribing responsibility will only transfer once dose is stable.

We do:

  • Initiate and titrate new medication only when clinically indicated

  • Provide clear titration plans aligned with HWICB Shared Care Guidance

  • Arrange ECGs at our Redditch HQ where required

We don’t:

  • Prescribe for patients already under GP Shared Care unless a change is required

Communication & Signposting

We believe in proactive and clear communication and signposting:

  • All reviews are recorded in your patient’s EMIS record

  • Urgent issues trigger an EMIS task to your practice

  • You’ll be notified if a patient disengages from the reviews process

  • Where new mental or physical health issues are identified, patients are signposted to a variety of local resources and services
